Funding Guidelines

Individual Grant Criteria
-The request must be for equipment or services related to the health of the individual.
-The individual must be a resident of Lebanon County.
-The Individual’s Household income must be at 250% of Poverty Level or below. As of 2025:
1 person – $39,125
2 persons in family – $52,875
3 persons in family – $66,625
4 persons in family – $80,375
-The need must be substantiated in writing by a medical professional.
-Two bids must be submitted by vendors for any equipment request.
-The VNA Foundation Individual Grant Request form must be completed with additional support, substantiated and bid information (as appropriate) attached.
-Any insurance or medical assistance coverage the individual has should have been pursued to cover the request prior to application to the VNA Foundation.
-Funding decisions are ultimately at the discretion of the VNA Foundation Board based on the availability of funds and funding priorities.
Agency Grant Criteria
-The agency must be based in Lebanon County and serve the citizens of Lebanon County.
-The agency must be a 501C3 non-profit agency registered with the Commonwealth of Pennsylvania.
-The request must address healthcare or health-related issues.
-The VNA Foundation does not fund ongoing general operating expenses or
administrative salaries.
-The VNA Foundation funds medical equipment, services and programs that are client-related.
-The VNA Foundation funds one-time grants on a yearly basis with no obligation for renewal.
-The VNA Foundation Agency Grant Request form must be completed with additional support, substantial and bid information (as appropriate) attached.
-The agency’s financial information and a substantiation of the need must be included with the grant application.
-Funding decisions are ultimately at the discretion of the VNA Foundation Board, based on the availability of funds and funding priorities.
