• Agency grant applications are due March 31st.

• Individual Household Income must be at 200% of Poverty Level or Below (as determined by the federal government guidelines). 2015 Federal Poverty Level Guidelines Current Guidelines:
  • 1 person – $23,540
  • 2 persons in family – $31,860
  • 3 persons in family – $40,180
  • 4 persons in family – $48,500
• Add $8,320 for each additional person.

• The request must be for equipment or services related to the health of the individual.

• The appropriate application form provides a general outline of the information you should include within a grant request. It is helpful to the governing Board, when evaluating a grant request, to have abundant detail within the grant request.

• By completing the appropriate grant request application form. An individual applying for a grant request should complete the Individual Grant Application. An agency or organization should complete an Agency/Organization Grant Application. Once the appropriate form is completed, you may submit the Application on our website, or you may mail the application – including 14 copies each of the Application and any attachments – to the mailing address below:
